For this rotation, I met on zoom with my site evaluator Dr. Saint Martin as well as my classmates who were also on their psychiatry rotation. Dr. Saint Martin gave each of us feedback that we could all learn from.
For my first site visit, I submitted an H and P about a patient who had PTSD and was admitted for inpatient psychiatric treatment s/p a self-aborted suicide attempt by overdose. I had followed this patient throughout her stay and learned a lot about treatment for PTSD and the approach to a patient with suicidal ideations. I also submitted 5 drug cards. I chose ones that I had seen used commonly on my rotation. These included: aripiprazole, escitalopram, lorazepam, valproate, and nortriptyline.
At my second site visit, I presented a case of a patient who had schizophrenia with additional cannabis use disorder. We discussed the differences between drug induced psychosis and schizophrenia. I learned from Dr. Saint Martin that when the disorder is primarily substance induced, the psychosis will resolve after some time while if it is due to schizophrenia, the psychotic episode will not resolve without the proper treatment. I also presented an article which did a retrospective analysis on different risk factors for psychosis with cannabis use.
In conclusion, my site visits with Dr. Saint Martin were valuable learning experiences. He gave useful feedback on my H and Ps and care plans that I will refer back to. I also appreciated hearing the different interesting cases that my classmates presented.
